S.E.M.I.: Side Effects May Include… is a single-player and co-op action platformer for PC. Up to four players can play together online. You play as patients trying to escape from a special clinic. The clinic is full of staff members and other patients who will try to stop you.

Players fight using close combat and physics-based attacks. You can also use special abilities to get past obstacles and traps. The game has rooms with deadly traps and puzzles. One wrong move can knock out your whole squad. Voice chat is useful for coordinating with your friends.

Pills are a big part of the game. Taking pills gives your character special abilities. Bipolar Force turns you into a strong monster for some time, then makes you completely helpless. Laser Astigmatism gives you two sights but only one laser, so aiming becomes very hard. Stendhal Syndrome makes your character go hysterical when looking at a painting, causing destruction around you. Alice’s Syndrome makes you very tiny or very big, and you must adjust fast.

There is also a Madness status in the game. When you or your friends go mad, you start running, jumping, and fighting without control. Your abilities also become uncontrollable during this state. Taking pills can help you return to normal.

The locations in S.E.M.I.: Side Effects May Include… are randomly generated. Rooms change with gravity shifts, moving traps, and other surprises. This means no two runs are exactly the same. The game supports both solo play and online co-op with up to three friends. The mix of brawling, pill abilities, and unpredictable rooms makes every session very chaotic and fun for groups.
